aryeh00 If your phone has the option, I would recommend formatting it in the phone (usually somewhere in the storage settings) that way it's optimized for the phone. If not then do it with a computer like Nhaaps said, you can keep the default settings, the format should be FAT32. And yes, it does erase everything, which is why I said to copy it to a computer first. If you format it with I computer, I would say to insert it into the phone before you copy it back on, that way the phone will set up the card with all its files and folders before
